Designed to support those at high risk of Type-2 diabetes. This CDC year-long program aims to help patients lose weight and make better dietary choices.
Chronic Care Management
This program help those with multiple chronic conditions expecting to last at least 12 months, including high blood pressure, Type-2 Diabetes, pre-diabetes, obesity and high body fat.
Behavioral Health Therapy
The Behavioral Health program seeks to identify and help change potentially self-destructive or unhealthy behaviors.
Medication Assisted Treatment
The treatment of substance use disorders with medications in combination with counseling and behavioral therapies.
